My impression during "Per Manum" (and maybe this was just my inherent shippiness, but I don't think so) was that his initial hesitation was due not to the fact that Scully was asking too much of him, but that she wasn't asking enough.
I think he really wants to form a family with Scully — Mommy, Daddy and Baby — okay, conceive the kid through IVF if that's the only way it's possible, but still have the relationship between Mommy and Daddy at the heart of the family. His line "I wouldn't want this to come between us" kind of hinted at that. But, because he loves Scully so much, he was willing to help her achieve what she wanted (motherhood) even if he didn't get what he wanted from her in return (marriage or, at the very least, mutual declarations of love and a sexual relationship).
We don't know exactly when the flashback parts of "PM" occurred, but I think it's safe to assume it happened after the events of "Triangle". So Mulder had already told Scully he loved her and had, basically, been rebuffed. I think he'll be happy to know the baby was conceived the old-fashioned way: now they've each got what they want.
