I still would like a little OnDeath script that determines what kind of injury the character suffers. It would be a random choice and should be pretty easy to make. For example:
25% chance of death
25% chance for a leg wound (slower movement for a little while)
20% arm wound (lower strenght/attack rolls)
20% torso (con loss? hp loss?)
10% head wound (int/wis loss)

Effects could be temporary, ranging from a few minutes to the entire session or could even be permanent to give it more effect.
This way death is still possible, yet does not happen -every- time you get into a fight. I've always found it silly that losing a fight automatically means you die in a game. Injuries are much more fun to RP as well.

After having played the official campaign some, I can say that death is actually pretty common.

On top of a little script I'd like to see some very rare resurrections. We could receive some kind of special reward item for a quest that allows us a single resurrection.

If a system like the above is used I vote for no ressurection spells. If we decide for another system I'd like to see some very limited resurrection.

I think there won't be a need to 'handle' magic. I believe the IC 'rules' against magic (like the permit inside the town) as well as the fear for it will be enough to keep any mages from casting too much magic.
I'd only like to see magic limited if it really becomes too common.

Full PvP and every item should be lootable (which means that every item will have to be flagged as such, unfortunately).

I'd like to keep the resting settings the same. As for time settings, I'd say that 15 minutes per IG hour sounds good enough.

At most there ought to be not more than 3 magic items in the entire campaign at one point. I'd like to see weapons a bit more expensive than they are in the SP campaign, or players simply need to own less gold.

The SP difficulty setting assumes you have hired a henchman, and I think it also assumes you use magic weapons. I'd prefer it if we don't use henchmen in Faalor and as few magic items as possible as well. As such you might want to watch out with any triggers that spawn creatures to any 'challenge ratings' or whatever they're called.

And Duath also needs a brothel like the Neverwinter City Core has!! :P
