I haven't read all the posts, but have you tried any kind of reward system? Of course, it doesn't have to be a monetary reward, just something that might encourage him. You probably know something that he might like to earn.....like reading a special book,a walk, or an ice cream cone. Just something you are sure he would really want to receive. Then, you and he draw a picture of whatever it is. Cut the picture into two or three puzzle- like pieces. Each time he goes to the potty properly, give him a piece of the puzzle to tape on the inside bathroom door. As soon as he earns all the pieces of the puzzle - he gets his reward. In this way, the appropriate behavior is rewarded and it becomes a positive experience. Of course, you will increase the amount of pieces gradually. Hope this helps.
