A few thoughts on this...I also have ceramic uppers and my canine brackets have always been tied to the archwire with tie wires, but they use a white teflon coated tie wire for that. They sometimes even add an elastic ligature over top of that. I don't see why your ortho couldn't use a teflon coated tie-wire.

Regarding the extra treatment time...I had a LOT of tooth movement with my canines in the first year - the uppers were each moved back an entire tooth length each, which amounts to about 1 mm per month. This is standard, safe practice and certainly an acceptable rate of movement regardless of the type of bracket. In my opinion, ceramics did not increase my treatment time. You might notice that I've been in braces for 28 months now...this is still within the original estimate, but I expect to go over. The ceramics on the canines were changed out for metal to provide better root torque, which I need in order to get implants. That root torque is what is holding the treatment back and doesn't have anything to do with the ceramic brackets.

Once I surpass 30 months in treatment, I will start paying $100 per month until the braces are removed. This was told to me up front and was on the contract that I signed before they put the braces on. It would be the same if I had all metal. I agree with you that you should have been told ahead of time any extra fees that you might have to pay.

Regarding the wait for a molar band... I doubt that it'll set your treatment back much, but I would like to have problems taken care of ASAP. I can usually always get same-day emergency appointments.
