She was seriously injured by a mob in Prague long-after Angel disappeared to be on his own. I don't remember the details being spelled out, but I believe it was recent and certainly in the 1990's.

In Becoming Part I, we hear Angel being cursed with the return of his soul in Romanian. Granted it's not proof positive, but Prague is not in Romania.
