Mr. L and I have always believed that her problems are physically-based. (She was premature and severely underweight at birth.) There's an excellent book called The Hyperactivity Hoax, by a prominent neurologist, which contains much supporting evidence for that belief.

However, my sister is a teacher, and they are thoroughly indoctrinated in all the ADD shit. My sister really believes she needs those meds, and none of the rest of our family can convince her otherwise. Do you know what they do, at the high school where my sis teaches, when the Ritalin kids get out of control? They send them down to the cafeteria for a cup of coffee. I shit you not. That is their policy.
