I agree with you that having my legs apart and being fully exposed is standard for the diaper position. It is how I have always gotten, both as a girl and now as an adult. And I think for the overwhelming majority of women and girls it is done that way. BUT I do believe that it is 100% always the case for everyone.

Awhile back I chatted with a young woman who describing how she gets a spanking from her parents in diaper position . She lays on her back and raises her legs up. One parent takes her feet and holds them together with her legs sticking straight up while the other parents administers the spanking to her butt and the backs of her thighs. She acknowledged that the way her parents do it is unusual but she is thankful for being saved the embarrassment of having to be fully exposed. An interesting, if unusual, method.
