I am new to the reborn doll art. I have found a very successful way to open the nostrils. I have the Pampered Chef cake tester. I heat it over an open gas stove flame. When it is hot (few seconds) I insert it into the center of the nose. The heat melts a small hole. I then reheat the metal to burn off the vinyl, (you will see an orange flame)
wipe off any soot with a paper towel and reheat and insert again. I really feel confident with this method because the instrument is very small and I can enlarge the hole a little at a time. I am very happy with the results.
