The trick is to put the nozzle thingy in your mouth before you turn the water pik on (or you'll be cleaning the bathroom mirror!) and keep your lips closed.
I use it after eating and before brushing, running the water stream over and under my brackets. It *really* gets food out and it makes my gums feel better, too.
I put mouthwash in the water (no more than a 50/50 mix, they say) or baking soda. It really leaves my mouth feeling clean -- I would never feel that my teeth were clean with just the toothbrush.
