I think, having finally been more-or-less forced to acknowledge that the MSR was a sexual/romantic relationship, rather than a business/platonic relationship, he deliberately withheld as many clues from us as possible, so that we'll never know exactly when "it" happened.
Also, the assumption on everybody's part seemed to be that they weren't lovers prior to the flashback events of "Per Manum", because in that episode she asked him to help her conceive a baby through artificial means. But did she ever *say* at any point in "PM", that she wasn't involved in a romantic relationship? Scully, Mulder and the doctor were all under the impression that it was impossible for her to conceive via the natural method; lots of married couple have to resort to aritficial insemination or IVF procedures to conceive their children (like the couples in "Small Potatoes") but one assumes they *also* have sexual intercourse with each other on a regular basis, for the fun of it!
Of course, since we were never told at exactly what point the flashback events of "PM" were occurring, that doesn't tell us much anyway, does it?
