In regards to a woman who is nursing:
Pregnant and breast-feeding women, if they fear for themselves or for the baby, can break the fast and pay the "ransom." They do not have to make up the days missed.
Abu Dawud related from 'Ikrimah that Ibn 'Abbas said concerning the 'ayah "And for those who can fast [but do not],": "This is a concession for the elderly, as they can fast. They are to break the fast and feed one poor person a day. Pregnant or breast-feeding women, if they fear for the child, can do likewise." This is related by al-Bazzar.
At the end of the report, there is the addition: "Ibn 'Abbas used to say to his wives who were pregnant: 'You are in the same situation as those who can fast [but do not]. You are to pay the "ransom" and do not have to make up the days later.' " Of its chain, ad-Daraqutni says it is sahih.
In regards to the woman who still has days to make up from the previous ramadhan yet she did not, she needs to make them up as soon as possible before the new Ramadhan begins.
For, to not make up the days of the previous ramadhan by then is a sin. If she cant fast, she must feed a poor person for each day she owes. If she does not make the fasts up before the new Ramadhan then she is a sinner and will have to seek repentance to Allah in addition to fasting and feeding a poor person for each day missed. (Majmoo' Fataawa al-Maqaalaat li'l-Shaykh Ibn Baaz)
In regards to ghusl, if one takes a ghusl then there is no need to follow up with Wudhu.
