I don't know if the requirements of vengeance demons are ever clarified. But at the end of season six in one of those episodes that all kind of blend together, when Xander finds out that Anya has her vengeance on again and tells Buffy, she asks, "Why don't you just go to her?" And Anya says "normally I'd have to, but..." The implication is that Willow is preventing her from it because Willow wants to do it herself. Also implied is that yes, Anya is under some form of obligation in these matters. Whether or not it would be to grant the wish, I don't know.

I don't share Celebailin's interpretation that Spike must have known all along what anya was up to, but I do think when she stops his wish, he might have caught on. I don't think the text is definitive either way, but his expression suggests to me the thought of, "whoa, you don't want me to make a wish." Of course, the fact that he doesn't follow after her saying, "I wish I didn't have this chip, I wish I didn't have this chip" could be an argument that he didn't know.
