Hello Gail,
As Tom has started you off in the right direction,i thought that these few pointers may assist as well.

If you go to the following site (Record Copying)

When page opens click on "Order FORM"

This will take you to the next step for ordering your paper copies once you have found them.

By the looks of what i have searched for the following Ref Number relates to

HYDE M C Ref BT372/146/57

So when the page opens for the Ref just type this in and then it should come up with the request this.
The Documents they send for request will have all the info you want.
Please try and supply as much info as you can in the necessary boxes!

It is a free service for requesting,and you will only have to pay if you want to continue,after you have received the requested info.

This is the direct link to that File,but you have to go through the channels above to order it.
