I just did a quick check and without any real effort I found these examples

1.3 The Witch

Amy's mom ends up bound in the statue.

1.8 I Robot, You Jane

The binding of Moloch into the book allows him to be scanned onto, and have freedom on, the modern data storage equivalent; the web.

2.8 The Dark Age

The summoned Demon Eyghon wants its payment from Ripper and the rest of the group.

2.16 Bewitched, Bothered and Bewildered

Amy's love spell for Xander goes wrong with potentially fatal consequences.

2.17 Passion

The gypsys' curse on Angelus backfires when Angel is de-souled and kills Jenny, the gypsy descendant.

3.9 The Wish

Anya's granting of Cordelia's wish nearly leads to vampires taking over the world and then results in Anya becoming dispowered instead.

3.11 Gingerbread

All magic and magical abilities become dangerous as MOO attempts to 'treat' the symptoms rather than the cause. Amy ends up a rat on a long-term basis (for having invoked Hecate once too often IMO).

3.13 The Zeppo

All the raised boys end up dying permanently in short order.

3.16 Doppelgangland

Anya and Willow's attempt to regain Anya's power centre results in death as Vamp Willow is accidentally brought into our dimension, Cordelia and Willow are both nearly killed.

3.20 The Prom

Tucker gets caught! Bit feeble this one.

3.22 Graduation Day Pt.II

It's Dino-demolition time, arguably due at least in part due to the failure of R.W. III to make the sacrifice in Band Candy.

4.4 Fear, Itself

The student accidental summoning has serious consequences both for them and ultimately for the Fear Demon.

4.9 Something Blue

Wilow's spell doesn't achieve her aim but her wishes do come true after a fashion with near fatal consequences.

4.12 A New Man

Ethan's last mistake nearly gets him killed but does get him caught.

4.17 Superstar

Johnathan's spell produces a 'Shadow Johnathan' which causes all the problems.

4.21 & 4.22 Primeval & Restless

The dreams of Restless are a consequence of the spell of Primeval.

5.11 Triangle

The existance of Olaf the Troll is a result of Anya's first act of vengence and it comes home to roost.

5.17 Forever

Dawn's ressurection spell places her in front of Doc and so at risk, it also raises a zombie Joyce.
