The handbook thing is a continuity issue that demands fanwanking of some sort.

The Prophecy Girl thing is the whole point. At that moment, when Buffy says "I know who you are," she defeats the prophecy and is granted a way out by whatever power rules over such things. And the reason is because Buffy goes willingly, and even arguably in the spirit of compassion (the boy says "help me") so, no longer in denial and resistance about her spiritual destiny, she is able to see all clearly for what it is.

Buffy's both too smart and too true for prophecies. That's why if she had read the same prophecy Wesley read, she wouldn't have stolen Angel's baby, she would've said, "I stand with Angel. The prophecy can do what it wants."

And consequently, she wouldn't have had her throat cut by treacherous bastards, or been suffocated by pillows later on.
